Developing innovative integrated care solutions for People with Multimorbidity (PMM)
Mål
CareMatrix is a European H2020 Pre-Commercial Procurement (PCP) project for integrated care solutions designed to challenge the health market to develop innovative technology for People with Multimorbidity (PMM).
PMM are aging populations who have two or more chronic health conditions such as diabetes, heart disease and depression.
The Integrated Care approach centres around person-centred, equitable, efficient, safe and coordinated care. The approach is proactive in supporting PMM to address both their health and social care needs. Symptoms based on complex illnesses interacting can then be planned for and treated holistically.
The CareMatrix buyers’ group supports solution providers financially and in a structured co-design process, to develop technologies that anticipate and adapt to the changing needs of PMM by ensuring timely service access, co-managed care journeys and transitions between specialties and sectors.
