Cheryl Matter, PhD, Vice President, Population Health, Careforth
At Careforth Cheryl Matter leads a team responsible for developing and implementing sustainable and scalable models of care that improve the health, well-being and experience of patients and their caregivers.
Cheryl has more than 15 years of experience advancing innovative healthcare solutions focused on addressing the needs and barriers of our most vulnerable and complex individuals.
Most recently, Cheryl served as the VP of Population Health at Lifesprk, a senior-focused home and community service organization where she was accountable for the development, tech-enablement and integration of offerings that include home-based primary care, care management, hospice, risk adjustment and partner management.
Prior to Lifesprk, Cheryl led the development of a tech-enabled community-based palliative care service for Livio Health, a BCBS MN sister organization and held a variety of clinical, business and leadership roles within emerging companies, accelerators, and industry support organizations.
Cheryl has a PhD in Molecular and Medical Pharmacology from UCLA.

Nathan Vogt is the Director of Healthcare Initiatives at GrandPad, a company with a mission to improve the lives of millions of seniors by reconnecting them with family, friends, and caregivers.
For over 15 years, he has been a versatile business leader in the Healthcare Technology Industry, known for successfully building business with large accounts and developing and sustaining strong partnerships. Specializing in value-based care, healthcare software solutions, business process improvements, and Medicare Advantage.
As the Director of Client Services for MMC 20/20, he led the division managing over a dozen Medicare Advantage clients, with over 10 million members, providing software solutions and business process improvements.
Previously, as Director of the Class Action Center at Stowell and Friedman, Ltd., he developed and managed a claims center for this nationally recognized civil rights law firm to enable successful litigation of a $400 million discrimination class action settlement.
He has a Master of Arts (MA) focused on Philosophy and Religious Studies from Harvard University, receiving both Williams and George Chase Honorary Fellowships.
